Clear Communication
We keep you informed throughout your patient's care. Consultation findings, treatment recommendations, and relevant records are shared with your office so we can stay aligned every step of the way.
Urgent cases seen urgently
Call the office directly for an acute abscess, a failing site or an unexpected finding and we will make room the same week wherever the schedule allows.
Your restorative plan leads
For implant cases we plan backwards from the restoration you intend to place. Tell us where the crown needs to sit and we will put the fixture where it belongs.
Your Patient Returns to You
We're here to complement your care, not replace it. Once periodontal or surgical treatment is complete, your patient returns to your office for restorative and ongoing dental care. When periodontal maintenance is needed, we coordinate with your hygiene schedule to provide seamless long-term care.

Fax it ahead, or send it with the patient. Anything missing we will ask you for, but a complete package means the consultation is a consultation rather than a data gathering exercise.

Mark faxed records for the attention of the doctor, with the patient name and date of birth on the cover sheet.

  1. Patient name, date of birth and contact number
  2. Reason for referral and the tooth or site in question
  3. Recent radiographs, including a full mouth series or CBCT if you have one
  4. A current periodontal chart if one has been taken
  5. Relevant medical history, current medications and known allergies
  6. Your preferred timeline, and whether the case is urgent

Urgency

For an acute abscess or a failing site, call the office as well. Marking a form urgent does not make the phone ring.

This form is a normal web form, not an encrypted channel. Send only what is needed to open the file and call the patient. Radiographs, periodontal charts, medical history and medications should come by fax to the numbers above, or with the patient.

It is also not watched around the clock, so it is not the way to reach us about an emergency. If your patient is experiencing a medical emergency, call 911.
