The problem in the upper back jaw
The maxillary sinus sits directly above the roots of the upper molars and premolars. When those teeth are lost the sinus tends to expand downwards into the space, and the ridge resorbs upwards to meet it. What is left can be a few millimeters of bone, which is not enough to hold an implant.
How the space is created
The membrane lining the floor of the sinus is carefully lifted and graft material is placed beneath it. Over the following months that graft turns into bone with enough height for an implant.
Where a small amount of lift is needed and there is enough existing bone to stabilize a fixture, the lift can be done through the implant site itself and the implant placed at the same visit. Where more height is needed, a lateral approach is used and the implant follows once the graft has matured.
Planning
This is a procedure planned from a cone beam CT scan rather than a flat radiograph, because the anatomy of the sinus, the position of its septa and the thickness of the remaining ridge all determine which approach is appropriate.
